Required Documentation

Students applying for enrollment in the Supported Learning Program must submit the following documentation to the program instructors in order to complete their application. Please submit all documentation to: Mary Ellen Schubert, Supported Learning Program, Davis & Elkins College, 100 Campus Drive, Elkins, WV 26241.

Report of current psychological/educational evaluation completed within the last two years indicating both ability and achievement levels. (A complete, confidential report that clearly states test results, a diagnosis and recommendations is necessary and also beneficial.)
Two letters of recommendation, one each from a teacher and counselor who have the best understanding of the student's abilities and needs.
Copy of the most recent IEP.
A campus visit including a personal meeting with one of the instructors from the SLP. This can be arranged by calling our Office of Admission at 304-637-1230 or 1-800-624-3157.

In addition to the materials above, please submit a handwritten essay that includes the following information:

How do you hope to use the Supported Learning Program at Davis & Elkins College?

Submission

Please Note: The application for the Supported Learning Program (SLP) is separate from your application to Davis & Elkins College. Also please note that no decision about acceptance to the SLP can be made until the College application process is complete.

I understand that I am required to submit two (2) Recommendation Forms, one (1) Report of Current Psychological/Educational Evaluation, one (1) Copy of the most recent IEP and one (1) Handwritten Essay in addition to this completed application. Upon sending this application, I certify that this information is true and correct to the best of my knowledge.
